Please add the names and birth dates of all members living in the home for six or more months of the year, including yourself. Include the Social Insurance Number (SIN) for all household members 18 and older.
This information is used to verify income with the Canada Revenue Agency. Please ensure each name is spelled exactly as it appears on the most recent tax return.
Note, for privacy reasons:
- Only the last three digits of a SIN will display once you click "Add".
- All information for household members under 18 years of age will be deleted from the application after the eligibility decision is made.
